Insurance Claim

1. Complete the Claim forms
and supporting documents
2. Submit all documents
to Prudential
3. Prudential will advise you
of the result after processing the claim

1. Hospital and Surgical Claims
- Required documents to begin a claim assessment.
1.  Duly completed Hospital and Surgical Claim Forms

2.  Hospital Discharge Note (a true copy certified by PVN Customer service staff )

3.  Hospital invoice (a true copy certified by PVN Customer service staff)

4.  Surgery certificate (a true copy certified by PVN Customer service staff)
- Required documents during the assessment period to provide sufficient information for claim decision (informed by Prudential if necessary)
1. Medical report (for surgery / Intensive care treatment / hospitalisation more than 15 days)
2. Death Claims
- Required documents to begin claim assessment
1. Duly completed Death Claim Form

2. Certified true copy of Death Certificate

3. Original Hospital Discharge Note

4. Original temporary receipt and Accident Report provided by the Police station (submitted only if the LA has deceased during the temporary insurance period)
- Required documents during the assessment period to provide sufficient information for claim decision (informed by Prudential if necessary)
1. Original Medical reports, medical test reports, prescriptions and imaging diagnosis.

2. Original Death notice

3. Original Police accident reports (for accidental death only)
- Required document to complete claim payment
1. Original Life Certificate (required when main Life Assured Passed away)

2. Copy of Payee’s ID card

3. If the policy has an appointed beneficiary and the beneficiary is under 18: an original Birth Certificate in case father/ mother of the beneficiary represents the beneficiary; or a certified true copy by the Legal Guardian Approval in case a beneficiary’s guardian represents him/her.

4. If the policy does not appoint any beneficiary: depending on the sum assured of the policy, the Prudential staff who receives the claim request will instruct the customer regarding the appropriate inheritance procedure as per the following:

-   An inheritance agreement issued by local authority or notary public; or

-   An agreement to appoint a representative payee certified by the local authority or notary public on the representative’s signature.

-   A document that declares the first inheritor and an agreement to appoint a representative payee certified by local authority or notary public on the representative payee’s signature

*** In case the Life Assured under 1 year passed away, Claimant just submit:

                                                Claim request + Death Certificate + Life Certificate
3. Total and Permanent Disability / Accidental Disability claims
- Required documents to begin claim assessment:
1. Duly completed Total and Permanent Disability / Accidental Disability Claim Form

2. Hospital Discharge Note (a true copy certified by PVN Customer service staff)
- Required documents during the assessment period to provide sufficient information for claim decision (informed by Prudential if necessary)
1. Original Medical Reports, medical test reports, prescriptions and imaging diagnosis.

2. Original Police accident reports (for accidental event)

3. Original Total & Permanent Disability appraisal issued by the Medical appraisal committee (at the National/Provincial/City level) at least six months after the date of the claim event.
- Required document to complete claim payment
1. Original Life Certificate (required when main Life Assured has been TPD)
4. Critical Illness benefit
- Required documents to begin claim assessment:
1. Duly completed Critical illness claim Form

2. Hospital Discharge Note (a true copy certified by PVN Customer service staff)
- Required documents during the assessment period to provide sufficient information for a claim decision (informed by Prudential if necessary)
1. Original Medical Reports (including Lab tests and diagnosis)

2. Original police accident reports (for accidents)
5. Pru Lady – child birth and child birthday benefit
- Required documents to begin claim assessment:
1. Duly Completed Pru Lady Benefit Claim Form

2. Certified true copy of birth certificate
6. Pru Lady – other special and core benefits for female
- Required documents to begin claim assessment:
1. Duly Completed Pru Lady Benefit Claim Form

2. Certified True Copy of Death Certificate (for death event)

3. Hospital Discharge Note (a true copy certified by PVN Customer service staff)
- Required documents during the assessment period of a claim request (when Prudential need more information)
1. Original Medical Reports (including Lab tests and diagnosis if any)

2. Original police accident reports (for accidental event)
